Position Overview
We are seeking a Full Time Registered Nurse to perform administrative services and care coordination for the Office of Community Care. Work will be provided on-site at the VA Office of Community Care Glendale, Colorado office.
Pay: $37hr-$49/hr
• Location: Office of Community Care (OCC 4100 E. Mississippi Avenue Glendale, Colorado
• Hours/Schedule: Full-Time, Onsite, Monday -Friday 7am – 5:30pm, excluding federal holidays
Key Responsibilities
• Provide coordination and management of patient care
• Educate patients and families about health conditions, treatments and self-management strategies
• Provide administrative functions with interdisciplinary teams
Qualifications
Required:
• Associates or bachelor’s in nursing
• Minimum of 2 years’ experience as a RN Case Manager or related field
• Active & Unrestricted State RN license
• [Security clearance requirements]
• BLS for Healthcare Professionals (AHA)
Preferred:
• Certification in Case Management (CCM or ACM)
• Strong communication and interpersonal skills
• Familiarity with electronic health records (EHR) systems
